Description: The Registered Nurse implements professional nursing practice centered on the patient as a whole person. The focus is on the creation of a caring environment centered on healing and wellness. Nursing practices are based on evidence, current research, and regulatory standards. Clinical decision-making incorporates all phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines. Coordinates, ensures, and provides direct patient care activities through assessments and treatments. Facilitates admission, transfer, and discharge activities. Provides patient and family-centered education and individualized treatment planning as applicable. Performs and documents comprehensive assessments in accordance with scope of practice. Considers the patient’s culture, age, and developmental level. Identifies patient risk factors and collecting health information as necessary. Develops a problem list and prioritizes patient needs based on input from the inter-professional team. Determines desired outcomes in coordination with the patient and family. Explains procedures, treatments, and processes to patients and families. Recognizes, responds to, and communicates changes in patient conditions appropriately. Conducts and documents ongoing assessments and responses to procedures and therapies. Delegates tasks appropriately and oversees team members in the performance of patient care. Identifies and addresses educational needs of patients. Facilitates the patient discharge process by executing discharge orders timely. Promotes a positive group dynamic and healing environment through words and actions. Actively participates in quality improvement activities and core measures. Uses a holistic perspective regarding patient care.
